Yes. A parent, guardian, or family member may complete the intake form using information provided by the student.

Does my child still have to sign the documents?

Yes. Because the student or adult child is the person granting legal authority, they must review and sign the completed documents before they become effective.

Are the documents notarized?

Some documents require notarization and/or witnesses depending on your state's laws. Instructions will be included with your completed package.

Is FERPA included?

Yes. FERPA Authorization is included for students attending or planning to attend college, university, trade school, or vocational school.

How long does it take?
